Waiver of Claim Release and Indemnification Agreement: In consideration of the acceptance of my entry in the “Canadian Tire Mitchell’s Bay Open”, I release the organizers (members of the MBAA), sponsors, volunteers, members of all other affiliated organizations, and any other person assisting in this event from any claims for damages or injuries suffered by me as a result of my participation in or traveling to or from the event. I hereby state that I am in proper physical condition to participate in this event and am aware that participation could, in some circumstances, result in physical injury. Having read the rules and regulations, I hereby submit my entry form for registration and enclose my full Entry Fee.
2023 Picture Waiver: I agree to the use of my name, portrait, picture, or photograph as part of the Canadian Tire Mitchell’s Bay Open image bank. This is a collection of images of anglers, volunteers, and sponsors involved in the Canadian Tire Mitchell’s Bay Open Bass Tournament and are intended to showcase the tournament’s activities.
I agree that I shall have no claim against the Mitchell’s Bay Area Association or corporate sponsor Canadian Tire or against anyone accessing or using images from the collection. I confirm that I am over 19 years of age and that I have not given anyone the exclusive right to my name, portrait, picture, or photograph.
By confirming your registration and submitting this form, I agree with the Waiver of Claim Release and Indemnification Agreement/Picture Waiver. I also agree to abide by the published Tournament Rules.
